The Abbott CardioSupport Program is a Pre- and Post-Implant Education Program for Your Patients who are eligible for the CardioMEMS HF System.
In order to ensure the smoothest learning experience, Abbott provides 1-on-1 support to patients and their caregivers to assist them throughout their pre- and post-implant experience.
Once you identify a patient for the CardioMEMS HF System, we recommend that they join Abbott’s CardioSupport Program.
Patient fills in the enrollment form and signs up for the program.

A dedicated team of customer service professionals, specially trained to educate your patients and provide:
Personalized Education
Provide individualized support to help patients understand the CardioMEMS HF System, prepare for their implant, and receive virtual assistance post-discharge in the comfort of their homes.
Direct Technical Assistance
Offer a direct line for CardioMEMS HF System technical support.
Patient Empowerment
Empower patients to take daily readings and use their CardioMEMS HF System with confidence.
Any clinical questions will be directed back to the patient's care team.

Pre-Implant Education
Within three business days of signing up, we will call your patient to provide supplementary education that is intended to support the information provided by healthcare professionals, including:
- What the CardioMEMS HF System is and how it works
- Understanding the basics of the procedure and transmitting data
- Living with the CardioMEMS HF System
- Additional details about how the CardioSupport program will support them before implant, day of the implant and after the implant
Post-Implant Education
Within three business days after the implant procedure, a representative from Abbott’s CardioSupport Program will reach out to your patient again. During this call, we will provide education on using the CardioMEMS HF System and ensure the patient can perform a successful reading.
CardioMEMS HF System Covered by Medicare Advantage
With the NCD (National Coverage Determination) for implantable pulmonary artery pressure sensors, more patients may have access to the CardioMEMS HF System if coverage criteria is met.
