Policy and Request Form

Abbott maintains a list of clinics that specialize in using Abbott’s implantable heart failure devices. This is provided as a courtesy and is accessible to the public. The clinic locators provide a list of clinics that are in geographical proximity (up to 200 miles) to the zip code or address entered by the user.

There is no cost to the user or the individual clinics. No direct connection is made by Abbott between a potential patient and a clinic.

The clinic locators are regularly updated for the intent of maintaining accuracy of addresses, locations, and other information as listed on the locators. Abbott routinely updates the locators according to eligibility criteria.

Any information you send to a clinic is not covered by our Privacy Policy.

Clinics who wish not to be listed on a locator, or clinics not listed on a locator, who wish to be listed, may complete this online form to express their request. Abbott will take necessary steps in response to the request to ensure the clinic's status is updated.
