Patient understands and consents that Abbott will use the above contact information to conduct outreach and deliver pre-implant educational information regarding the CardioMEMS HF System.
Patient has been provided a copy of this form, which includes QR code for Abbott’s Privacy Policy and Consumer Health Data Policy. Patient can opt-out of the pre-implant program from Abbott at any time by informing the CardioSupport education team, following the opt-out instructions in the post-enrollment confirmation email, or following directions provided in Abbott’s Privacy Policy under “Your Privacy Rights and Choices”.
Patient understands that participation in Abbott‑provided education may involve the discussion of sensitive information, such as health‑related information shared during an education call. Abbott may use information from these educational interactions, in de‑identified form and in accordance with applicable U.S. privacy laws, to analyze and monitor the CardioSupport program for quality and improvement purposes.
