Electrophysiology

NICE Guideline [NG196] publication on the diagnosis and management of atrial fibrillation.

The National Institute of Health and Care Excellence (NICE) recommended that point‑by‑point RF ablation should be considered if drug treatment is unsuccessful, unsuitable or not tolerated in people with symptomatic paroxysmal or persistent atrial fibrillation.

One of the reasons this recommendation was made was that point‑by‑point RF ablation was found to more cost-effective than cryoballoon ablation and other approaches to ablation.

Cardiac Rhythm Management

NICE Technology Appraisal of implantable cardioverter defibrillators and cardiac resynchronisation therapy for arrhythmias and heart failure.

ICDs and CRTs are recommended as a treatment options. The guidance defines the patient groups.
 

Heart Failure

Guidance from NICE: Interventional Procedures Guidance (IPG 711) on percutaneous implantation of pulmonary artery pressure sensors for monitoring treatment of chronic heart failure.

This guidance recommends standard arrangements are in place for glinical governance, consent, and audit on pulmonary artery pressure sensors for monitoring treatment of chronic heart failure.

Publication of cost-effectiveness of HeartMate 3 vs. medical therapy in transplant-ineligible patients

This peer-reviewed manuscript supports HeartMate 3™ LVAD in destination therapy as a cost-effective option vs. medical therapy in the UK for advanced heart failure patients ineligible for transplant.

NICE Interventional Procedures Guidance on percutaneous occlusion of the left atrial appendage in non-valvular atrial fibrillation for the prevention of thromboembolism.

These recommendations include normal arrangements for clinical governance, consent and audit or research in the procedure of percutaneous occlusion of the left atrial appendage in non-valvular atrial fibrillation for the prevention of thromboembolism.
 

NICE Interventional Procedures Guidance on percutaneous closure of patent foramen ovale to prevent recurrent cerebral embolic events.

These recommendations include normal arrangements for clinical governance, consent and audit or research in the procedure of percutaneous closure of patent foramen ovale to prevent recurrent cerebral embolic events.

NICE Interventional Procedure Guidance on percutaneous mechanical thrombectomy for acute deep vein thrombosis of the leg

The recommendations include:

For acute iliofemoral DVT, the evidence on efficacy is limited in quality and quantity, therefore this procedure should only be used with special arrangements for clinical governance, consent, and audit or research.

For distal DVT that does not extend into the common femoral vein, the evidence on efficacy is inconclusive, therefore this procedure should only be used in the context of research.

These recommendations include special arrangements for clinical governance, consent and audit or research in the procedure of percutaneous insertion of a temporary heart pump for left ventricular haemodynamic support in high-risk percutaneous coronary interventions.
