The experts deliver one-on-one education to help a patient understand how to use their remote monitoring product and the importance of staying connected — all from the comfort of the patient's home.

- The HRS, EHRA, LAHRS, and APHRS expert consensus on practical management of the remote device clinic states that patients should be educated about the importance of maintaining ongoing connectivity for appropriate remote monitoring.2
- Support services can reduce the time patients need to stay post-procedure for remote monitoring setup and education.

A Smooth Setup and Patient Experience
Patients are enrolled in Merlin.net™ Patient Care Network (PCN). Abbott SyncUP Remote Monitoring support experts contact patients after enrollment.

The patient is given a brochure letting them know they will be contacted by an Abbott SyncUP support expert. The expert will educate the patients on remote monitoring and assist them with setting up their remote monitoring product. Patients should watch for a call from Abbott SyncUP.

Within five days of receiving the request from the clinic, an Abbott SyncUP Remote Monitoring support expert will work with your patient to set up their remote monitoring product. The expert will:
- Assess if the patient needs an Abbott transmitter or has the required product already
- Explain how to use the remote monitoring equipment and educate the patient about remote monitoring
- Answer questions related to remote monitoring
- Assist with a transmission to your clinic, if needed
