CardioMEMS HF System monitors heart failure progression by providing real-time data about your heart's pressures so your doctor can make proactive and informed decisions about your treatment.

You are being considered for the CardioMEMS HF System because your care team thinks it can help to manage your heart failure and you qualify based on the fact that you:

• Have NYHA Class II or III heart failure

• Have been hospitalized for heart failure in the past 12 months and/or

• Have an elevated BNP or NT/pro-BNP

Here are a few things you should know when considering the CardioMEMS HF System:

How is the CardioMEMS HF System different?

In the past, a patient's condition was determined by how they felt at the time. Now,  your doctor no longer has to wait for you to feel bad or for symptoms to appear, which can be too late. The CardioMEMS HF System data can indicate worsening heart failure weeks before symptoms appear and is proven to reduce heart failure hospitalizations and improve patient-reported quality of life.2

Here's how the system works:

During a common surgical procedure, a tiny pressure-sensing device, about the size of a dime, is placed in your pulmonary artery (PA).

 

The patient electronics system is used to take daily PA pressure readings from anywhere with a connection. A reading only takes 2-3 minutes.

 

The system sends these readings to your care team through a secure website. Your care team reviews the readings and contacts you if any changes are needed.
